Helpful Reminders for Completing Your Application
  • Signatures must be completed via the eApplication electronic signature process or wet signature on paper applications. Signatures completed via unapproved electronic signature software (i.e. DocuSign, PDF Complete, etc.) will not be accepted. 
  • The "Section B continued" field of the Part 2 should only be used to provide additional details for "Yes" answers from Section A.  Please do not include any extraneous information (i.e. My client exercises regularly) not directly related to questions in Section A as it causes unnecessary review and processing delays.
